Listed prices are for Zelle payments. Credit card payments made through the health record system (ClinikEHR) include the processor fee of 2.9% + $0.30 per transaction.

Florida residents, located in Florida at visit time
- You must be physically located in Florida during your visit.
- Visits are self-pay only; no insurance is billed.
- Teenagers with a parent or guardian present, through elderly adults.
- Not treated: controlled substances, emergencies, or injuries needing an in-person exam.
